Chronic Venous Disease (CVD) Epidemiology, Prevalence, Incidence and Progression

Afflicting over 190 million U.S. citizens, chronic venous disease (CVD) is one of the most common chronic diseases. CVD prevalence dwarfs that of other chronic diseases including diabetes (38 million), peripheral artery disease (22 million) and all cancers combined (18 million). Despite this, CVD is underestimated, underdiagnosed and undertreated.

The report focuses on CVD epidemiology, prevalence, incidence and the percent progression to more severe disease. The primary purpose of this research is to estimate the number of U.S. adults with CVD, the number with chronic venous insufficiency (CVI) and the number in each of the C1-C6 CEAP classes.

Because venous disease is frequently bilateral, CVD, CVI and C class estimates are provided in number of limbs, as well as in patients.

Prevalence and incidence percentages employed for the calculations are based on the conclusions reached after a comprehensive literature review. These percentages are applied to the U.S. Census Bureau’s 2020-2040 population projections for the population ages 18 and older.

Prevalence and incidence estimates for the 2020-2040 period are provided in 5-year increments.

Because the CEAP classification system provides a standardized disease definition, studies reviewed are limited to those that employed this system. Due to regional differences in prevalence and incidence, studies are restricted to those conducted in Europe or the U.S. Studies reviewed were also limited to those that examined populations with a broad age range, including both younger and older age groups.

Published disease progression data is limited to CVI (C3-C6), varicose veins (C2), edema (C3) and skin changes due to venous disease (C4). When available, we calculated disease progression by applying the percentage of patients progressing in a CEAP class to the 2025 prevalence in that CEAP class.
